When applying for Defense Base Act Insurance, we will need a completed signed application that we will e-mail to you and a copy of the Statement of Work (SOW).
We request that you complete both an electronic version of the application and also print one that can be signed and faxed to our office or scanned and sent to us electronically. Please do not hesitate to contact us by e-mail or by phone with any questions.
